How to change date format to MMM-YYYY

I am trying to create a description that says this: Jan-2025 Accrual.

I used the Create Date & Time Range node to enter the date, but I would like to change this format to MMM-YYYY.,

I would also like to make this a consistent description so that it is there for every row. In the end I want the workflow to look like this:

How do I achieve this?
